Host-Home

In Arizona MENTOR's Host-Home program, children or adults with intellectual and developmental disabilities live in private family homes within the community provided by a network of local caregivers – who we call Mentors.   The Mentor family supports the individual in acquiring skills and participating in community life according to her or his own interests and abilities. Because this model is geared towards the needs of one person, host-home living is highly flexible and focused on personal choice for the Mentor and the individual receiving services. In partnership with the people we serve and our Mentors we strive to create that “perfect match,” ensuring successful placements that lead to long-term, meaningful relationships – the basis of our program success.

Together the Mentor, program staff, the individuals’ family and other professionals design and implement an Individualized Service Plan (ISP) to help those we serve live life to the fullest and participate in the daily rhythms of neighborhood life. Our Mentors provide services to address each individual’s needs while developing personal relationships that provide the foundation for growth.  Every individual in our Host-Home program is actively involved in planning their daily routine and the services they need to learn new skills and behaviors.  With the right supports and services, we believe individuals facing a range of challenges have the best opportunity to realize their full potential in the communities they call home.
