In-Home Support
Arizona MENTOR's In-Home Support Services are designed to help individuals achieve their greatest level of independence and offer a flexible alternative to group homes and other residential options. In this program, we design individualized support options using a comprehensive life planning process that focuses on personal choice and allows individuals with developmental disabilities to control how, where and with whom they wish to live. Every individual we serve participates in the process of selecting their own services and developing their own life goals. In addition, we work collaboratively with individuals and family members to select direct service professionals to best meet each person’s needs and preferences.
Our trained staff work with each program participant to help them become more independent and provide assistance with housekeeping, personal care and other daily activities, habilitation and social skills development, as well as access to neighborhood resources and transportation. Our services support each person’s personal and physical needs including:
Attendant/Personal Care
- Personal hygiene
- Transfer to and from wheelchairs and/or bed
- Meal preparation and nutrition assistance
- Assisting with daily activities
Habilitation
Habilitation services prepare individuals with disabilities to live independently in the community by developing functional skills and behavior alternative strategies. This service provides training in self-care, communication, socialization, physical and emotional expression, cognitive functioning and problem-solving, pre-vocational skills and leisure activities. Arizona MENTOR’s Habilitation services are designed to enable each individual we serve to be a valued member of his or her community.
Housekeeping
Housekeeping services provide safe and sanitary living conditions for the individual’s personal space or common areas of the home that the individual shares or uses.
Respite
Respite services provide short or long term care and supervision to individuals. Respite can be performed either in or away from the individual’s home. The goal of this service is to provide a break for the primary caregiver, while maintaining the social, emotional and physical needs of the client.
