Promoting Independence, Respecting Choice

Arizona MENTOR's In-Home Support Services help individuals achieve their greatest level of independence and offer a flexible alternative to group homes and other residential options. With support from Arizona MENTOR staff, the individuals, their families and guardians actively participate in creating an Individualized Service Plan (ISP) tailored to their unique needs and goals.

Through personalized support options developed using the ISP, we help individuals with intellectual and developmental disabilities live as independently as possible in the home and community of their choice. Our In-Home Support program includes the following supports.

Habilitation services prepare individuals with disabilities to live as independently as possible in the community by developing functional skills and behavior alternative strategies. This service provides skill development in self care, communication, socialization, physical and emotional expression, cognitive functioning, as well as problem solving, pre-vocational skills and support participating in leisure activities.  Arizona MENTOR’s habilitation services are designed to enable each individual we serve to live life to the fullest in his or her community.

Attendant Care

Attendant Care services provide personal care while meeting the essential personal and physical needs of individuals we serve and assisting with the tasks necessary for their comfort and safety.

Respite services provide short-term care and supervision for the individuals we serve, either in or away from their home. The goal of this service is to provide a break for the primary caregiver, while ensuring that the social, emotional and physical needs of the individual served are met.

Housekeeping services provide safe and sanitary living conditions in the individual’s home. Supports provided may vary depending on the family’s needs but can include services such as cleaning and help changing bed linens.
