How We Help

clientGuitarArizona MENTOR’s continuum of person-centered services provide adults and children with intellectual and developmental disabilities and other complex challenges with the supports they need to live fully and more independently in the communities of their choosing. Our individualized programs are tailored to each person’s unique needs, respecting their goals and preferences for service and including input from their families or other significant people in their lives.  Services are carefully monitored in order to make adjustments as needs change or new milestones are met.

A Continuum of Personalized Services

Arizona MENTOR’s person-centered services and supports include:

  • Individualized service plans
  • Case management
  • Clinical supports
  • Life skills development
  • Structured activities
  • Community integration
  • On-call support
  • Advocacy services
  • Transportation
  • Quality assurance
  • Program evaluations
  • Relief support
  • Family support

Quality services depend on resources, expertise and quick response times—all of which are readily available at Arizona MENTOR.  As a part of Sevita—previously The MENTOR Network, a national network of local human services providers—we are able to draw upon more than 40 years of knowledge and an array of experience in successfully serving people in home- and community-based settings.
