Community Living Residences

ClientGroupArizona MENTOR believes that a home environment is where people thrive.  When they feel comfortable and safe at home—surrounded by people who care about them—they have greater opportunities to learn, grow and overcome challenges.

In our Community Living Residences, also known as group homes, two or more adults with intellectual and developmental disabilities are carefully matched based on their needs, preferences and compatibility to live in our licensed, community-based homes.

Comprehensive Support

Arizona MENTOR goes beyond the “traditional” set of services offered in group home settings. We provide intensive services for adults with intellectual and developmental disabilities as well as complex medical and behavioral challenges and focus on creating meaningful connections between staff and residents. The Community Living program includes:

  • 24-hour home-based monitoring by specially trained, full-time staff
  • Medication management
  • Community integration
  • Structured activities
  • Independent living skill development
  • Help with daily living tasks such as meal preparation
  • Transportation
  • 24/7 on-call support from our team of experts

Each Arizona MENTOR Community Living Residence is unique, offering a range of amenities including:

  • Private, spacious rooms
  • Comfortable furniture
  • Full modern kitchens
  • Accessible bathrooms tailored to each resident’s mobility needs
  • Personalized service and support

Individualized Services

AdultServices

We provide all individuals in our homes with a level of care designed to help them feel at home and succeed.  Depending on each individual’s needs, our support services may include assistance with:

  • Personal hygiene
  • Cooking and housekeeping
  • Accessing community resources
  • Participating in community events
  • Transportation
  • Nursing care
  • Help with budgeting and managing finances

Specially Trained Staff

When someone is ready to move into one of our residences, we take the time to understand what interests them as well as their unique strengths and abilities.  Our homes are staffed with Direct Support Professionals and other human services staff who develop Individualized Service Plans (ISPs) to help each person progress toward their goals and succeed—whatever that means for them.  We make sure that individuals move into homes suited for their unique needs and do our best to match them based on age, gender and level of care.
